Complete a calibrated warm-versus-cool detection session under clinical control

Thermal discrimination is noticing a controlled change from a declared starting temperature at a named body site. A qualified clinician uses calibrated equipment, protects the skin, randomises trials and interprets thresholds. The learner still learns the response method, reports the first clear change and checks that the configuration stays constant. There is no home heat, ice or tolerance test.

1 · Permission and limits

Know exactly what you may do

You may

  • Prepare a symptom and skin-history record, learn the response button and stop at any time.
  • Ask the clinician to explain the body site, starting temperature, rate, limits and interpretation.

Qualified help is required for

  • Screening, equipment calibration, stimulus limits, trial order, skin inspection, measurement and interpretation.

2 · Get ready

Gather what you need and check the starting conditions

What you need

  • A clinic-approved calibrated thermal sensory device with a contact thermode—a temperature-controlled skin probe—and automatic safety limits.
  • Provider protocol, response button, skin inspection supplies and a settings/result sheet.
  • A stable supported position and the learner’s usual communication and mobility aids.

Before you start

  • Tell the clinician about skin damage, altered sensation, circulation issues, recent injury, medication changes and prior adverse reactions.
  • The clinician confirms the exact site is intact and suitable and explains warm detection, cool detection and the stop response.
  • Agree a plain stop word or button that ends a trial immediately without needing to justify it.

3 · The method

Follow these steps in order

  1. Declare the clinical question

Scenario

A clinician assesses warm and cool detection at the intact skin of the left forearm with a calibrated contact thermode.

Walkthrough

  1. The clinician documents the forearm site, equipment, 32°C start and protocol limits after screening.
  2. Sam practises pressing at the first slight warm or cool change and repeats the rule in his own words.
  3. He completes a random block containing warm, cool and catch trials while the clinician records each response.
  4. After the block Sam reports no pain or lingering sensation; the clinician inspects the skin and continues once.
  5. The clinician reports separate warm and cool detection values with protocol variability and explains that they do not show burn safety.

Result

The session produces interpretable results for the left forearm under that device and protocol. It does not establish a safe household temperature, a pain threshold or general thermal ability.

Turn the steps into a usable skill

First session

  1. Provider screening and skin check.

Repeat plan

Testing or practice frequency, trial count and any repeat are set by the qualified clinician for the declared purpose. A repeat uses the same site, device, starting temperature, rate, response rule and rest unless the change is explicitly the question.

8 · Check the result

Measure what changed

Provider-calculated warm and cool detection results at one named body site.

How: Use the calibrated device protocol to record response temperature for each valid trial, catch-trial behaviour, within-session spread and symptoms; the clinician applies the declared summary rule.

Good result: The provider judges familiarisation adequate and valid trials sufficiently consistent for the stated clinical question, without adverse skin or sensory effects.
