Recover from a protected perturbation with a step, not a fall drill
Reactive balance is the response after support or body position changes unexpectedly. This lesson teaches a protected laboratory method: stand in a provider-fitted harness, keep a natural ready stance, allow a small platform or treadmill perturbation, take one or more recovery steps toward the direction of the loss, regain a stable base and report whether the harness, rails or provider had to catch body weight. The provider controls eligibility, harness, perturbation size/direction, trial count and progression.

What you need
- Certified overhead safety harness and provider-checked attachment
- Calibrated perturbation treadmill or platform with emergency stop
- Guard rails positioned as declared
- Video or trial sheet for step, catch, rail and symptom scoring
- A protected-balance log for harness fit and test, perturbation direction and magnitude, timing, recovery steps, rail contact, catches, assistance, symptoms and reset.
Before you start
- Confirm the harness supports body weight without contacting the learner during ordinary standing.
- Practise the stop signal, emergency stop and safe return to the platform.
- Report falls, faintness, vestibular symptoms, pain, fear, neurological change and any condition affecting safe stepping.
- Fit and load-test the harness according to its instructions, then practise one guarded weight shift. Proceed only if the system catches the shift without strap pain, breathing obstruction or unsafe slack.
- Practise the natural ready stance, one supported weight shift and the instruction to respond rather than guess before unpredictable trials begin.
3 · The method
Follow these steps in order
- Fit and test protection
Let the provider fit the harness, clear straps from the neck and test the support over the platform before any disturbance.
Why: A harness is protection only when fitted and attached correctly.
Check: The attachment catches a controlled weight shift without strap pain or obstruction.
- Set a natural ready stance
Stand on the marked feet with the usual footwear and authorised aid configuration, arms relaxed and gaze at the fixed target.
Why: A repeatable stance changes the available recovery step.
Check: Feet, aid, gaze and rail rule match the trial card.
- Respond rather than predict
Wait for the provider's signal that a trial may occur, but do not count down or pre-step. Let the small perturbation start before moving.
Why: Pre-planning removes the reactive element; this preserves the reactive balance rule for 'Respond rather than predict'.
Check: Video shows no step before platform movement.
- Step toward the loss
If balance moves beyond the feet, take a quick step in that direction and use additional steps as needed; do not cross the legs unless that is the taught response.
Why: A recovery step enlarges the base under the moving body.
Check: The new foot lands where it supports the displaced body.
- Regain a stable base
Bring the other foot to a comfortable stance, steady the trunk and hold for two seconds before the next trial.
Why: The response is incomplete until balance is restored.
Check: No continued hopping, rail grab or harness loading occurs during the hold.
- Name any assistance
State whether you used a rail, the harness took weight or the provider touched you. Do not hide assistance to make the trial look successful.
Why: Assistance changes the outcome and safety interpretation.
Check: Learner and provider agree on the assistance code.
- Reset and vary only by plan
Return to the exact foot marks and rest. The provider changes only the predeclared magnitude or direction and stops at the first failed safety rule.
Why: Unplanned escalation can exceed stepping capacity.
Check: Every trial has a known setting and full reset.
4 · Worked example
See the whole method used once
Scenario
A balance specialist uses an overhead harness and six low-magnitude treadmill-belt slips: two backward, two forward and two lateral, in a predeclared pseudorandom order.
Walkthrough
- The harness is fitted and tested; the learner practises a forward, back and side recovery step without belt movement.
- Trials one to three end with one or two steps and no rail or harness loading.
- Trial four requires a rail touch; it is recorded as assisted, not a failure hidden from the result.
- Trial five produces harness loading, so the provider stops the series and does not deliver trial six.
- The log records direction, magnitude, steps, rail, harness catch, fear and symptoms for every delivered trial.
Result
Three unassisted recoveries, one rail-assisted recovery and one harness catch were observed under protected laboratory perturbations. This does not prove fewer community falls or permit home perturbation practice.

7 · Practice
Turn the steps into a usable skill
First session
- Provider assesses readiness and fits/tests the harness.
- Practise stepping and stable reset without perturbation.
- Complete one or two fully announced very small familiarisation perturbations.
- Complete a short provider-selected protected series with rest.
- Review step direction, assistance, fear, symptoms and stopping decision.
Repeat plan
Only the qualified balance team selects perturbation direction, magnitude, unpredictability, trials and sessions. Progress one dimension at a time; home trips, shoves and slips are never assigned.
Progress when
- Recovery begins only after the perturbation and steps move toward the displacement.
- Stable base returns without rail, harness or provider support across the declared set.
- Fear and symptoms remain acceptable and any community outcome is tracked separately.
Do not progress when
- A harness catch, fall-like event, rail dependence or unsafe crossing step occurs.
- Dizziness, pain, panic, neurological change or worsening gait appears.
- Harness, device, staffing or emergency procedures cannot be verified.
8 · Check the result
Measure what changed
Falls or harness catches and recovery-step quality during standardised protected perturbations, with community falls separate
How: Record device, stance, aid, direction, magnitude, timing, steps, rail use, harness loading, provider contact, symptoms and follow-up falls.
Good result: Across the declared protected set, recoveries become stable with fewer catches/assists and no worse symptoms; community transfer is claimed only if prospectively measured.

Accessibility and adaptations
- Keep the learner's usual aid when the provider can safely integrate it and treat aid use as part of the configuration.
- Use accessible consent and stop signals; adjust visual target, cue modality and rest without reducing harness protection.
- Use a tactile harness check, stance footprint and agreed verbal or light signal for setup, but keep perturbation direction genuinely unrehearsed and log all assistance.
