Scan, step and stabilise across one declared uneven route

Terrain adaptation is safe adjustment of gaze, step placement, aid use and speed to a specified surface. This lesson teaches a provider-built route with one firm uneven mat and one low obstacle: preview the route, choose the aid and lead strategy, approach at controlled speed, shorten steps, place the whole foot on known surfaces, clear the obstacle without rushing and pause after each transition. The provider controls surface, obstacle, guarding, repetitions and progression.

1 · Permission and limits

Know exactly what you may do

You may

  • Terrain adaptation: Stop on level ground and scan from near to far, naming the mat, obstacle, exit and any support point. Use the provider-agreed aid placement and lead foot for this route; keep the aid inside the support area.
  • During an authorised attempt, On the known uneven mat, place enough of the foot to support weight and keep knee/trunk aligned; use the aid before transferring weight as taught. Take one or two settling steps and pause if needed before the next feature; do not link features faster than control allows.
  • Point out each surface transition and exit before moving, then record obstacle contact, pause, stumble, assistance, time and symptoms for the declared route.

Qualified help is required for

  • Review mobility, vision, sensation, aid use and fall history and decide whether the secured indoor terrain route and guarding level are appropriate.
  • Select and secure terrain/obstacles, assess aid and lead strategy, guard the learner, set speed/trials/rest and decide any outdoor transfer.
  • Secure the mat and obstacle, set the usual aid and lead strategy, guard each transition and cue alternating preview gaze with foot placement instead of continuous downward staring.

2 · Get ready

Gather what you need and check the starting conditions

What you need

  • Provider-secured route with level lead-in/out, one uneven mat and one low obstacle
  • Usual mobility aid and footwear
  • Gait belt or guarding equipment when indicated
  • High-contrast edge marks and route video/score sheet
  • A terrain-route record with surface and obstacle dimensions, aid, lead strategy, guarding, contacts, pauses, stumbles, assistance, time, symptoms and invalid trials.

Before you start

  • Walk or visually inspect the route with the provider and identify every surface change and rest point.
  • Check that mats and obstacles cannot slide and that an escape route remains clear.
  • Report falls, dizziness, pain, sensory loss, vision change, fatigue and any new aid or footwear.
  • Walk beside the secured route and identify every surface change, obstacle and exit. Ask the learner to point out the next transition while standing still before attempting it with the planned aid and guard.
  • Walk the route beside the obstacles, rehearse the aid and lead plan and practise looking ahead before attempting the secured transitions.

3 · The method

Follow these steps in order

  1. Preview the route

    Stop on level ground and scan from near to far, naming the mat, obstacle, exit and any support point.

    Why: Planning reduces late surprise without removing the adaptation task.

    Check: The learner points to each transition and exit.

  2. Set aid and lead plan

    Use the provider-agreed aid placement and lead foot for this route; keep the aid inside the support area.

    Why: Aid and lead strategy affect stability.

    Check: The learner demonstrates the first transition on level ground.

  3. Approach under control

    Walk at a pace that allows a stop before the transition and shorten the final steps rather than lunging.

    Why: A controllable approach preserves time to place the foot.

    Check: The learner can pause before the mat without a recovery step.

  4. Place the whole foot

    On the known uneven mat, place enough of the foot to support weight and keep knee/trunk aligned; use the aid before transferring weight as taught.

    Why: Edge or partial contacts can slip or roll.

    Check: Foot and aid land inside the marked safe area.

  5. Clear the low obstacle

    Look ahead to the landing spot, lift the lead foot over without circling, then bring the trailing foot through before accelerating.

    Why: A rushed trailing foot commonly contacts the obstacle.

  6. Stabilise after each change

    Take one or two settling steps and pause if needed before the next feature; do not link features faster than control allows.

  7. Score errors and assistance

    Record obstacle contacts, foot-placement errors, pauses, rail/helper contact, time and symptoms. Keep the aid visible in the result.

4 · Worked example

See the whole method used once

Scenario

A physiotherapist builds an 8 m indoor route: 2 m level, 2 m firm uneven mat, a 5 cm foam obstacle and 3 m level exit. The learner uses a cane.

Walkthrough

  1. The learner previews all features and rehearses cane placement before the mat.
  2. Trial one contacts the obstacle with the trailing foot and requires a pause; no fall occurs.
  3. After feedback and rest, trial two uses shorter approach steps, clears both feet and pauses safely after the mat.
  4. Trial three repeats the route with no contact and the same cane, taking 14.2 s.
  5. The provider records errors, pauses and cane use; no outdoor transfer is claimed.

Result

The learner completed this secured indoor route with the cane and fewer obstacle errors. It does not establish independent travel on unknown, wet, steep or crowded terrain.

7 · Practice

Turn the steps into a usable skill

First session

  1. Provider inspects and demonstrates the route.
  2. Rehearse scan, aid placement and obstacle clearance on level ground.
  3. Complete one guarded familiarisation at slow speed.
  4. Complete two or three declared-route trials with rest and error feedback.
  5. Review errors, assistance, time and symptoms; no surprise terrain is added.

Repeat plan

The provider changes only one feature—surface compliance, obstacle height, route length, pace or aid demand—after repeated control. Outdoor, stair, slope and low-light routes require separate assessment.

Progress when

  • Foot and aid placement remain inside the safe zones.
  • Obstacle contacts and recovery steps reduce without aid removal or rushing.
  • The learner can stop before any feature and reports no new symptoms.

Do not progress when

  • A fall, near-fall, repeated contact, foot drag or unexpected assistance occurs.
  • Pain, dizziness, fear, fatigue or sensory/vision change appears.
  • The surface, obstacle, lighting, guard or aid cannot be kept constant.

8 · Check the result

Measure what changed

Errors, stability, time and assistance on one specified terrain route

How: Declare surface, obstacle, route, light, footwear, aid, guarding and pace; count contacts, placement errors, pauses, stumbles, assistance and symptoms.

Good result: At least two trials complete with fewer or no errors and stable aid use, without faster pace causing worse control.

This does not prove: It does not prove safe independent travel on every surface, outdoor community mobility or fall prevention.

9 · Stop, adapt or get help

  • Leave the route after a fall, new dizziness, unexpected foot or limb pain, or an aid failure; move to a safe surface and arrange the needed mobility or medical review.

Accessibility and adaptations

  • Keep the usual cane, walker, prosthesis or orthosis and record it as part of the route.
  • Use high-contrast, tactile or audible edge cues and accessible preview while preserving the physical route.
  • Preview terrain hazards with a tactile map, high-contrast route card or spoken sequence, recording guide, aid and cue choices without removing the tested surface changes.
