Appendix F: Non-Scoring Observation and Retest Templates

Use only the fields that are relevant to a declared task or chosen outcome. Leaving a field blank, declining assessment, pausing, or using an accessibility-equivalent route is valid. Titan requires no daily log, weekly grade, monthly score or ninety-day sprint.

Declaration — Record the task or outcome, why it matters, who chose it, the capability locus, the direct criterion, the evidence boundary and any professional owner.

Conditions and configuration — Record date or review window, setting, equipment or software version, assistance, accessibility supports, relevant constraints and whether the result is unaided or supported.

Direct result — Record the observed task result, unit, uncertainty or minimal detectable change where known, and what the result does not establish.

Retention and transfer — Record these only when predeclared and required. A delayed retest and a different transfer task remain separate results; neither may be inferred from immediate practice.

Burden and safety — Record time, cost, fatigue, pain or symptoms, adverse events, stop-rule use, privacy or consent concerns, and whether the route should continue, change, pause or retire.

Non-assessment reason — Declined, unavailable, contraindicated, inaccessible, not required, not interpretable and not yet due are complete states, not missing scores.

Review and retest — Record the human-chosen next action, owner and retest date. The Cockpit may show due, overdue, never retested or lapsed currency; it may not rank the person or choose a programme.

No total — Do not average, weight, grade or rank results across domains, capability loci or life areas. Do not produce an overall day, week, sprint, wellness, recovery, relationship or performance score.

Health observations such as sleep, nutrition, mood, symptoms, movement, heart rate or laboratory values remain optional, separate and context-dependent. Titan sets no universal target and does not turn them into a diagnosis or a composite result.
